Privacy Policy
Effective Date: August 30, 2024
At WholeHealthMD, your privacy and the security of your Protected Health Information (PHI) are of utmost importance to us. This Privacy Policy outlines how we handle your PHI, your rights regarding your health information, and how you can contact us with any concerns.
How We Handle Protected
Health Information (PHI)
WholeHealthMD uses and shares your PHI in accordance with HIPAA guidelines to provide you with healthcare services, process payments, and manage our healthcare operations. We ensure that all PHI is handled securely and confidentially. Your information is used in the following ways:
- Treatment: We use your PHI to provide medical care. For example, a doctor treating you for an
injury may need to know if you have other health conditions. - Payment: We use your PHI to bill and receive payment for the healthcare services provided.
For example, we may share your PHI with your health insurance company to get payment for
the services we provide. - Healthcare Operations: We use your PHI to improve our healthcare services and manage our
practice. For example, we may use your health information to review the quality of care you
receive.
Your Rights Regarding Your Protected Health Information
You have the following rights concerning your PHI:
- Right to Access: You can ask to see or get an electronic or paper copy of your medical record
and other health information we have about you. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee. - Right to Correct: You can ask us to correct health information about you that you think is incorrect or incomplete. We may say “no” to your request, but we’ll tell you why in writing within 60 days.
- Right to Request Confidential Communications: You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address. We will say “yes” to all reasonable requests.
- Right to Request Restrictions: You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no” if it would affect your care.
- Right to Receive an Accounting of Disclosures: You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why.
- Right to a Copy of This Privacy Notice: You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.
- Right to Choose Someone to Act for You: If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.

Regarding compounded semaglutide, our company works with Sterile compounding pharmacies. These facilities undergo strict inspections, but because the compounded pharmaceuticals they produce are made according to a prescription from a specific practitioner for a particular patient, they are not FDA-approved. It might be challenging to directly compare the results and risk profiles of FDA-approved and compounded pharmaceuticals due to differences in strength, dose, and other factors. The medical judgment of a licensed clinician, based on a telemedicine interaction and medical history, determines the prescription of any compounded medication.
